INSTRUCTIONS FOR JOB INJURY REPORTING
- If the injury is life-threatening, call 911 immediately. Then notify the District Risk Management Office at (760) 291-3238 and inform the employee’s supervisor.
- For all other injuries or illnesses, call the Injury Hotline (Company Nurse) at (833) 469-3184 right away. A nurse will assess the situation and advise on appropriate care.
- If medical treatment is needed, the employee will be referred to an authorized occupational medical facility. If no treatment is required, only the “Employee’s Report of Work Injury/Illness” form must be completed and submitted.
- If medical care is provided, the full Workers’ Compensation claim packet must be completed. Submit all documents by fax to (760) 291-1915, email [email protected].
- After every doctor’s visit, employees must provide updated work status reports to their supervisor. Those with a pre-designated physician on file may see their own doctor; contact Risk Management at (760) 291-3238 to confirm.
- Site secretary or supervisor must notify Risk Management immediately of the injury via phone or email, including the employee’s name, site location, and date of injury.
These procedures apply to all injuries, 24 hours a day, 7 days a week.
