Medical
Management
If your child has a medical condition or physical limitation for which school accommodations need to be made or needs to take medication at school, please ask your child’s California licensed provider / prescriber for the necessary documentation for our health staff to assist your child. We provide forms if the provider prefers to fill out a form. See links on this page. Note: new orders are needed for each school year.

Please provide the forms before the first day of school along with any medication or health device your child may need at school.
